Check Immunization Status

The State of North Carolina requires that all * students enrolled at UNCG during the Fall and/or Spring semester(s) have appropriate immunizations. It is best if you can get your immunization form completed by your health care provider BEFORE you begin the semester. Failure to complete the immunization requirements may prevent disbursement of financial aid. If the immunization requirements are not met dismissal from UNCG is mandatory under the law.

* There are certain provisions under which students may not be required to complete the Immunization portion of the Student Medical Form and are exempt from the Immunization Requirements. Please read the front page of the form for details. Students attending Summer Session only are not required to complete immunization/medical forms.

Please read the Student Medical Forms AND the NC Immunization Guidelines carefully. There are sections which must be completed by you, the student, and other sections which must be completed by your health care provider. If you remain continuously enrolled at UNCG you do not have to complete the Student Medical Forms for each semester. If you drop out for any semester other than summer session you should contact the Immunization Office at 336.334.4086 to determine if you need to re-submit the form.

If you have not met the immunization requirements, or are determined to be exempt for the semester within 30 days of the start of classes, you will be sent a warning letter stating you must comply or be dismissed from UNCG. If you get one of these letters IMMEDIATELY follow up with the Student Health Center.
